Temujanji Vaksinasi

LifeCare akan menjadualkan temujanji anda tertakluk pada ketersediaan stok.

Apa Perlu Saya Jangka?

  1. Sila minta Pegawai Syarikat anda untuk mengisi Borang Pertanyaan di bawah.
  2. LifeCare akan menghantar invois di bawah Nama Syarikat anda melalui e-mel (tertakluk kepada ketersediaan stok).
  3. Selepas pembayaran selesai, LifeCare akan kongsikan templat Senarai Nama Standard. Sila kembalikan kepada wakil LifeCare untuk pendaftaran.
  4. Untuk vaksinasi di premis syarikat sendiri, minimum 100 orang diperlukan.

Terma & Syarat

  1. Anda ditegaskan untuk mengikut Tarikh dan Masa janji temu. 
  2. Janji temu vaksinasi untuk dos kedua akan diberikan secara automatik berpandukan tarikh dos pertama.
  3. Pesanan akan diproses 1-2 hari bekerja setelah pembayaran berjaya. Hari bekerja tidak termasuk Sabtu, Ahad & Cuti Umum.
  4. Pastikan semua butiran syarikat adalah betul sebelum menghantar. Tidak ada perubahan butiran yang boleh dibuat setelah invois dikeluarkan.
  5. Pembayaran mesti dilakukan dalam masa 24 jam. Pembayaran separa tidak dibenarkan.
  6. Selepas pembayaran selesai, LifeCare akan kongsikan templat Senarai Nama Standard. Sila kembalikan kepada wakil LifeCare untuk pendaftaran. Pastikan semua maklumat peribadi betul sebelum menghantar senarai Nama.
  7. Minimum 100 orang diperlukan untuk vaksinasi di premis syarikat sendiri. Pusat Perubatan Diagnostik LifeCare akan menetapkan tarikh vaksinasi dalam 2 minggu setelah pembayaran berjaya bergantung kepada ketersediaan stok.
  8. Syarikat bertanggungjawab untuk mencari pengganti yang tidak hadir dan menggunakan sisa vaksin yang tidak digunakan.
  9. Setiap transaksi vaksin COVID-19 yang telah lengkap tidak akan dibatalkan, ditukar atau dibayar balik atas apa jua kondisi. Stok vaksin yang dijual adalah tertakluk kepada ketersediaan. Semua permintaan untuk pembatalan, penukaran atau pembayaran balik tidak akan dilayan.
  10. Pusat Perubatan Diagnostik LifeCare berhak untuk mengubah terma & syarat ini pada bila-bila masa dan tanpa notis.
  11. Pusat Perubatan Diagnostik LifeCare berhak untuk membatalkan pembelian Vaksinasi COVID-19 jika pembelian tidak dilakukan oleh syarikat yang sah atau untuk pembelian peribadi.

  2. LifeCare akan menghantar invois melalui e-mel (tertakluk pada ketersediaan stok).
  3. Borang Daftar Nama Piawai (akan dikongsi oleh LifeCare) untuk dihantar setelah pembayaran dilunaskan.
  4. Untuk Vaksinasi Di Tempat, minimum 100 orang diperlukan.

Terma & Syarat

  1. Ikuti Tarikh dan Masa Temujanji anda dengan tegas. Tolong sampai 15 minit sebelum Waktu Temujanji anda.
  2. Vaksinasi di lokasi yang akan dilakukan di syarikat anda sendiri mempunyai minimum 100 orang.
  3. Pembayaran mesti dilakukan dalam masa 24 jam. Tidak dibenarkan pembayaran separa.
  4. Pesanan akan diproses 1-2 hari bekerja setelah pembayaran berjaya. Hari bekerja tidak termasuk Sabtu, Ahad & Cuti Umum.
  5. Anda diminta memberikan senarai nama kepada orang Jualan LifeCare setelah pembayaran selesai.
  6. Pelantikan vaksinasi bergantung kepada ketersediaan stok vaksin. LifeCare akan menjadualkan temujanji anda dengan sewajarnya.
  7. Semua transaksi vaksinasi setelah selesai tidak dapat dibatalkan, ditukar, atau dikembalikan dalam keadaan apa pun.

    • Purchase 2 doses (per person) of CoronaVac COVID-19 Vaccine (Sinovac) @RM###  (Price inclusive of vaccination and consumables)
      1. One transaction can purchase up to 20 doses (10 persons) of COVID-19 Vaccine (Sinovac) ONLY
      2. You must schedule your vaccination appointment within 14 days after the purchase date as the appointment link (in the email) expires thereafter.
      3. Your first dose must be vaccinated within 90 days after purchase date. LifeCare will ensure appointment slots are available within 30 days from the purchase date.
      4. Please note that second dose of vaccination appointment is auto-generated based on your first dose appointment
      5. You are allowed 1x change of your 1st dose appointment only. Rescheduling of appointment to be done ONE (1) day in advance.

2. What Should I Bring?

  • Your original copy of Identification Card or Passport
  • An email confirmation with LifeCare COVID-19 Vaccination Appointment Card (barcode)

3. What Should I Wear?

  • You may opt for a t-shirt with a sleeve that can be easily folded up for the vaccination and a comfortable pair of pants
  • We will advise to bring along a jacket as it might get cold while waiting in the hall

  1. Can a person get COVID-19 from the vaccine?
    No. None of the approved vaccines contain live SARS COV-2 virus, so they cannot cause COVID-19 illness. Vaccines induce your immune system to recognise and fight off disease, but they do not actually cause an infection.
  2. Who should take the COVID-19 Vaccine?
    • Individuals who are 18 years of age and older
    • Individuals who are suffering from the conditions listed below (high-risk of suffering from severe COVID-19 disease if infected):
      • Type I or Type II diabetes
      • Obesity (BMI >40 with obesity-related disease)
      • High blood pressure
      • High cholesterol level in blood
      • Heart diseases (congenital, atrial fibrillation, coronary artery disease & heart failure
      • Lung diseases (asthma, chronic obstructive pulmonary disease)
      • Cancer (treatment completed & disease stable)
      • Chronic kidney disease (Stages 3-5, undergoing dialysis)
    • Vaccination is also safe and recommended for:
      • After acute illness: Fever (2 weeks after recovery)
      • Other vaccines (2 weeks after receiving other vaccines)
      • Mild allergies sufferers (food, medicine, insects bites – rashes, itchiness, swelling)
      • HIV patients (stable conditions)
      • Was infected with COVID-19 (3-6 months after recovery)
  3. What are the possible side effects of the COVID-19 vaccine? Will a person feel unwell after vaccination?
    The side effects may include pain, redness, swelling and itchiness where the vaccine was given. Some people experience local injection site reactions within 1-2 days after the vaccine, but they are usually resolved itself. Other side effects include tiredness, headache, fever, chills, muscle or joint soreness, nausea and vomiting. Most people feel those side effects slightly more after the second dose.
  4. Are vaccine side effects a good sign?
    The side effects are part of the immune response to the vaccine. However, everyone’s reaction to the vaccine is different, so the absence of side effects after vaccination does not mean the vaccine is not working.

        • On blood-thinning medications who are under observation and with scheduled follow-up appointments
      • Weakened immune system due to medical conditions or treatments
        • Cancer patients currently undergoing treatment
        • Individuals with organ transplantation history
        • Individuals with chronic HIV infection
        • Individuals on immunosuppressants
        • Individuals with autoimmune diseases (SLE and rheumatoid arthritis on specialised medication)
      • Severe allergy reactions (anaphylaxis)*
        • Towards vaccines, medications or shots that may contain polyethylene glycol (PEG) or polysorbate

        • After receiving the first dose within 72 hours

  1. Who is eligible to receive CoronaVac COVID-19 Vaccine (Sinovac)    
    • Individuals who are 18 years of age and older are eligible to receive the vaccine.
  2. Who is not eligible to receive CoronaVac COVID-19 Vaccine (Sinovac)
    • Individuals who are having at least one of the conditions listed below are not eligible to receive the vaccine:
    • Who are known to have allergic to any of the ingredients in the vaccine (Aluminium hydroxide, disodium hydrogen phosphate, monosodium dihydrogen phosphate, sodium chloride, sodium hydroxide); 
    • Who are known to have allergic to any other vaccines
    • With severe neurological conditions (e.g. transverse myelitis, Guillain-Barré syndrome, demyelinating diseases); 
    • With uncontrolled severe chronic diseases; 
    • Pregnant and breastfeeding women
  3. How is CoronaVac COVID-19 Vaccine (Sinovac) given?
    • The vaccine is injected into the deltoid muscle (shoulder) as a course of 2 doses, with 3 weeks (21 days) interval.
  4. What should I do after I am vaccinated?
    • You are required to wait at the observation area for at least 15-30 minutes after the injection
    • Please inform our medical team or get emergency treatment if you face these symptoms after the injection:
      • Breathing difficulties
      • Severe swollen of face, lips and throat
      • Rapid or fast heartbeat
      • Bad rashes on the whole body
      • Weakness and feeling dizzy
    • If needed, you may take Paracetamol
    • You should continue to follow the SOP. Practice the 3Ws (Wash, Wear, Warn) and avoid the 3Cs (Crowded space, Confined space, Close conversation)
    • Please report any side effects after receiving COVID-19 vaccine through MySejahtera application or inform the nearest clinic or hospital immediately to receive treatment. You can also make a report online at the website at www.npra.gov.my.
