Hypertension, also known as high or raised blood pressure, is a condition in which the blood vessels have persistently raised pressure. Blood is carried from the heart to all parts of the body in the vessels. When the heart beats each time, it pumps blood into the vessels.

Blood pressure is created by the force of blood pushing against the walls of blood vessels (arteries) as it is pumped by the heart. The higher the pressure, the harder the heart has to pump. Untreated high blood pressure can lead to serious consequences such as heart attacks, strokes, and other cardiovascular diseases.

Based on the National Health and Morbidity Survey (NHMS) 2019 by the Ministry of Health (MOH), more adults suffered from hypertension at 6.4 million people in Malaysia, or 3 out of 10 people (30%), with only half of them aware that they have high blood pressure. In 2015, the NHMS recorded 6.1 million adults with hypertension at a similar prevalence rate of 30.3%. Among those below 30 years of age, hypertension occurs at 90%, of which 45% have their blood pressure controlled.

The prevalence of hypertension among the population was found to be high, which indicates the need for hypertension-screening programs. Practical solutions including lifestyle interventions also need to be developed as there is a close relationship between obesity and various diseases, including hypertension.

Managing HBP with ECP

External Counterpulsation (ECP) is a non-invasive and well-established method for treating ischemic heart disease. It is the process whereby timed compression of the blood vessels to the rhythm of your heartbeat enhances blood flow back to the heart and throughout the body.

As blood flow increases during ECP Therapy, it causes friction of the flowing blood on the thin membrane that lines the inside of the heart and blood vessels to produce growth factors to have vasodilation (dilates the blood vessel) properties.

On the other hand, ECP therapy can help with arterial stiffness related to high blood pressure. Stimulation of the blood flow towards the blood vessel wall aids in the formation of nitric oxide which is responsible for the relaxation of the blood vessel.

In conclusion, heart workload will be decreased after the completion of ECP treatment, although it is increased during a 60-min ECP therapy. ECP may serve an alternative way to improve blood pressure after the course of ECP therapy.
