Receive Redemption Email
After submission, you will receive an email with instructions to download the LifeCare App, together with a unique referral code.
Download & Register on LifeCare App
Download the LifeCare App and complete the registration process.
Enter Referral Code
In the LifeCare App, go to My Profile > Referral Code, then enter the referral code provided in the email.
Voucher Issuance
Upon successful entry of the referral code, the LifeCare Proactive Health Pass voucher will be issued to your app account.
Make an Appointment
Book your appointment directly via the LifeCare App at your preferred LifeCare centre.
Redeem at Centre
Present the QR code in the LifeCare App during registration at the centre for verification and redemption.

Terms & Conditions

The complimentary Health Pass is available exclusively via LifeCare app.
Campaign period: 1 Feb to 30 Apr 2026.
Redemption validity: 31 Jul 2026.
The Health Pass is non-transferable, non-exchangeable, and not redeemable for cash.
Redemption is valid at the following LifeCare centres only:
1. LifeCare Diagnostic Medical Centre, Bangsar South
2. LifeCare Diagnostic Medical Centre, Cheras South
3. Loopa AI Functional Training, LifeCare Wellness, Bangsar South
Prior appointment is required and is subject to availability.
Each eligible user is entitled to one (1) redemption only.
