An applicant under 18 years of age MUST be accompanied by their parent or by a legal guardian. Parent/Guardian MUST present a government-issued ID or passport. In case the companion is not the parent or legal guardian, the following documents MUST be presented:
- Letter of Authorization stating that the companion is authorized to sign on behalf of the parents.
- 1 photocopy of the parent’s government issued ID or passport.
- Authorized companion’s government-issued ID, original and 1 copy.
If applicable, please bring the following:
- Previous chest x-ray film or report.
- A detailed medical report regarding a previously treated or currently managed clinically significant illness which should contain the following information:
  - Duration and date of treatment.
  - The surgical procedure or medical intervention done.
  - Complete final diagnosis.
  - The outcome of treatment or treatment plans and prognosis, if available.
ONLINE PURCHASE: present a copy of your invoice as a proof of payment upon registration.
Please provide a printed copy of your digital vaccination certificate (those who are 12 years and above).

REMINDERS:

For Blood & Urine Test – Fasting is not required.
Drink plenty of water before your urine test, so you can give an adequate urine sample.
Bring your spectacles or contact lenses.
Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the completion of menstruation.

Permanent Visa Applicants 2 -10 years old are REQUIRED to undertake either :

TUBERCULIN SKIN TEST (TST) requires 2 clinic visits. Second visit is 72 hours after the test (3^rd day). To avoid TST from becoming invalidated, please be at clinic at least 30 mins before the schedule written on your TST return slip.

The UK Pre-Departure Tuberculosis Detection Programme Medical Certificate will be issued on the same day if Tuberculosis screening is normal.
If you are 16 years old & below, you must be accompanied by your parent.
It is advisable for applicants to do their Tuberculosis screening at least 10 weeks prior to their application because if Chest X-Ray findings are suggestive of pulmonary tuberculosis, they will have to do a further test with Sputum Smears and Culture.
Applicants with the previous history of Pulmonary Tuberculosis is also required to do Sputum Smears and Culture.
The result of Sputum Smears and Culture will take up to 10 weeks.
The certificate will be issued to applicants whose results are negative. If results are positive, the applicant will be referred for treatment for a period of at least 6 months.
